UK only: How do you go about putting someone into a home?

My mum is terminally ill and I can no longer provide the care she needs. I also have an autistic son and caring for my mum doesn't allow me the time to give my son which he dearly needs. So my question is how would you go about putting her into a home? She is registered as disabled and get's DLA also.
